The Barrie and Community Family Health Team (BCFHT) is one of the largest family health teams in Ontario. We are comprised of Physicians, Registered Nurses, Nurse Practitioners, Dietitians, Diabetes and Respiratory Educators, Mental Health Counsellors, Social Workers, Occupational Therapists, Pharmacist, Foot Care Nurse and an Administrative team. Our Team is focused on improving access to comprehensive, patient-centered, team-based primary health care, which supports self-management, emphasizes health promotion and disease prevention, and enhances the management of individuals with chronic diseases through programs that are well linked with other local and community services.
The BCFHT is currently searching for a:
Registered Nurse/Registered Practical Nurse (Aging Well and STOP Programs)
0.6 FTE, Permanent
The BCFHT has an immediate existing vacancy for either a Registered Nurse or Registered Practical Nurse to work within our Aging Well Clinic and our Smoking Cessation (STOP) program. The Aging Well Clinic provides specialized care to community dwelling seniors with complex medical conditions, cognitive impairment and/or clinical frailty. As part of the interdisciplinary team, the RN/RPN works collaboratively with patients and care partners to support cognition, function, connections with community resources and to facilitate future planning. System navigation and linking patients with community services is an essential component of this nursing role.
Our STOP (Smoking Treatment for Ontario Patients) program is a province wide initiative that provides counselling and nicotine replacement therapy to those interested in smoking cessation or reduction and will involve 0.2 FTE per week of this posting.
Main Responsibilities
- Perform nursing assessments including but not limited to initial screening, chart reviews, MMSE/MoCA and other related geriatric screening
- Collaborate with clinic team members to ensure optimal care is provided for both patients and families
- Communicate health updates and information to patients, families, and other care providers as needed
- Provide education and support to patients and families
- Provide system navigation to patient & families
- Provide counselling, guidance, nicotine replacement therapy and other support resources to patients interested in smoking cessation or reduction, using the STOP program
Education, Skills and Requirements
- An RN with a Bachelors Degree in Nursing or RPN Diploma
- Current registration with the College of Nurses of Ontario (CNO)
- Minimum of 3 years experience in a related nursing role or family practice setting
- Experience working with seniors and geriatric patients
- Must have or be willing to be certified in formal smoking cessation training such as TEACH (Training Enhancement in Applied Counselling and Health)
- Strong working knowledge of computer applications such as MS Excel, Word and Outlook
- Experience using electronic medical records systems (experience with the Accuro EMR system considered an asset)
- Demonstrated team-based care and client centred approach to care
Additional Job Requirements
- Must adhere to all BCFHT Policies and Procedures
- Must be legally entitled to work in Canada
- Must be able to travel within the community
We offer Group Benefits as well as HOOPP (Health Care of Ontario Pension Plan) to those eligible.
