Aging Well Clinic

The Aging Well Clinic is dedicated to helping seniors living at home, or in retirement residences, deal with the complicated medical conditions that can develop with age.

The Aging Well Clinic uses an interprofessional approach. This means multiple health care providers work together to provide you with the best possible care.  

Services are covered through OHIP and include:

All clients of the Aging Well Clinic must be accompanied by a family member or companion who is familiar with your concerns.  The Aging Well Clinic works in partnership with you, your family member/companion and your family physician.  Once your medical conditions are stable, your medical care will be returned to your family physician.

How to Arrange an Assessment at our Clinic

If your family physician or nurse practitioner is part of the Barrie and Community Family Health Team, make an appointment with them to discuss your concerns. They can then make a referral to our clinic. 

We are unable to take appointment requests directly from patients or family members or for patients referred by health care providers who are not part of the Barrie and Community Family Health Team.

What to Bring to Each Appointment

Your First Appointment

You and your family member or companion will meet with members of our team.  Your first appointment lasts 1.5-2 hours.  This comprehensive assessment includes cognitive testing, a review of your social history, medical history, family history, medications, ability to complete everyday tasks and a physical check up.

Follow-up Appointments

Once your first appointment is completed, you will be advised of the next steps.  Most patients return to our clinic for additional appointments. These appointments can range from every month to every 12 months. Your appointments may be scheduled with any member of our team.  Due to the expertise of our team members, not all patients will require a visit with an Aging Well Clinic physician.

Our Team

All of our team members play an important role in your care.

Our Administrative Assistant will schedule your appointments and help direct your questions and concerns to the right person.

Our Nurse performs the testing used to diagnose memory problems and will speak regularly with you and your family to gather information, assess medications and offer support with any concerns or problems.

Our Nurse Practitioner independently manages medical conditions including memory problems.  The Nurse Practitioner can prescribe medication, order tests, and direct your care.

Our Occupational Therapist provides support to clients who due to chronic conditions, illness or injury are experiencing difficulty with everyday tasks.

Our Pharmacist has specialized knowledge about medications and can provide education and assistance with medication issues.

Our Physicians work part-time in the clinic to provide additional medical support to clinic patients.
