Who We Are

The Barrie & Community Family Health Team Clinical Pharmacists work within the Family Physicians' offices. Clinical Pharmacists work collaboratively with Family Physicians and other interdisciplinary health care professionals to ensure the most appropriate use of medications. Clinical Pharmacists are able to see people with a broad range of medication issues. Patients see the Pharmacist for one or more appointments as needed, and can be re-referred at any time.

Pharmacy Services Overview

The Pharmacist will discuss prescription medications, over-the-counter medications and herbal or natural remedies. They will work with you and your health care practitioner to resolve and/or prevent any health care problems.

A patient may be seen by a Pharmacist if:

  • Help is needed with medications for a chronic condition (e.g. diabetes, hypertension, cholesterol, COPD)
  • Taking multiple medications (to simplify, ensure appropriate dosing time, manage or prevent drug-related problems)
  • Experiencing an adverse drug reaction or is at high risk for adverse drug reaction
  • Recently been discharged from hospital with medication changes
  • Medication adherence issues

Pharmacists may also:

  • Provide drug information and education to physicians and patients
  • Assist with drug coverage issues
  • Liaise with community and hospital Pharmacists as necessary
  • Support illness prevention and health promotion through education and advocacy
